Need on Home-Based Occupational Therapy for Persons With Stroke

ABSTRACT

Objective : This study is purposed to measure needs on home-based occupational therapy of stroke patient and develop a home-based occupational therapy model.Methods : Subjects in this study were 168 patients(74 inpatients and 94 outpatients) diagnosed with CVA. 37 questions with each 4 answer choices were developed by the author and used as a tool for measuring the needs on the home-based occupational therapy. MBI(Modified Barthel Index) and MMSE(Mini Mental State Examination) were employed for the evaluation tools for ADL and cognitive function respectively.Results : 147 patients out of 168 patients answered that they need the home-based occupational therapy. 69 out of 74 inpatients(93.2%) and 78 out of 94 outpatients(84.8%) answered they were in need of the home-based occupational therapy. Average scores of MBI and MMSE were 63 and 24 respectively. The more were patients disabled, the higher need on the home-based occupational therapy were they. Disablement was categorized as speech impairment(p=0.0127), cognitive impairment(p=0.0001), visual impairment(p=0.0001), bladder(p=0.0278), bowel impairment(p=0.0010), dysphagia(p=0.0001), and disable glade(p=0.0001).Conclusion : This result shows that the CVA patients are in high demands for the home-based occupational therapy to adapt themselves to the lives at home and society. The home-based occupational therapist system has not prepared yet, however, in-depth studies shall be further carried out to well establish the home-based occupational therapy Model and occupational therapist system.
